Care Coordinator

At Prudent Domiciliary Care, we provide elderly citizens with quality care that enables them to live happier, healthier lives in their own homes.

We are looking to hire a committed and experienced Care Coordinator to join our friendly team. The successful candidate must be organised, efficient, and driven to help the team maintain a high standard of service. This position is for a part-time role, consisting of three days per week, scheduled between Monday to Friday, with additional on-call duties on a pro-rota basis. There may be occasions where the candidate will be required to work full-time to cover colleagues’ holidays/days off. Furthermore, please note that the role can convert to a full-time position.

Requirements

What we’re looking for:

  • Applicants must have a basic understanding of medication administration and client needs
  • Applicants must have at least 2 years of carer experience, and ideally experience working in a supervisor role
  • Excellent communicator at all levels, both verbal and written

Duties and Responsibilities

Duties & Responsibilities:

  • The Coordinator will undertake risk assessments of the working environment for our elderly clients and the Prudent Domiciliary Care staff, including the elderly client’s homes and all equipment used by them.
  • To support the care staff and assess/undertake conduct and competency observations. This includes but is not limited to undertaking the relevant feedback, review and investigations to ensure smooth delivery, at the request of the Care Manager.
  • You will be supporting the Care Manager with investigations, fact-finding and document retrievals in the event of any complaints, safeguarding or unsatisfactory service deliveries.
  • Maintain accurate and up-to-date records for customer electronic files
  • Liaising with GP’s, District Nurses, Social Services, and other Health Professionals
  • Expected to participate in the on-call as per weekly/daily rota
  • Taking an active part in on-call and emergency care responsibilities as required
  • To comply with all company policies and procedures
  • Prepare new customer information packs and distribute
  • Supervise and monitor staff to ensure they are meeting the company standards of care delivery and adherence to safeguarding and mental capacity policy and procedure
  • Carry out any other duties deemed necessary for the successful operation of the business
